Health News for the 2012-2013 School Year

For all Parents and Guardians:

Physical and/or Dental examinations are required if your child is entering the following grade levels for the 2010-2011 school year;

• Kindergarten and/or 1st grade - Physical and Dental Examinations
• 3rd and 7th grades - Dental Examinations
• 6th Grade and 11th grades - Physical Examinations
• 7th Grade - immunizations: Tdap and MCV   

28 PA Code 23.2(a)* states "Medical examinations...shall be provided on original entry into school, in grade six and in grade 11."

28 PA Code 23.3(a)* states "Dental examinations shall be required on original entry into school and in grades three and seven."

These reports must be completed by a physician or dentist and dated on or after June 16, 2010. Please return to the school as soon as possible.

Forms have been sent home with the students.  If you are in need of an additional copy, please contact the school health office.

The UPMC Care Mobile and Duquesne Family Health Clinic have forms on hand for students being seen there.
